Medical Appointments During Evacuation

Patient Information and Support Contacts

Patient Responsibilities

  • Notify the host site Liaison Officer or Health Lead of any upcoming medical appointments
  • Contact Ininew Patient Services (IPS) upon arrival at the evacuation site and provide current location and contact information (e.g., cell phone number).
  • If possible, sign the electronic consent form to receive Notices of Appointment (NOAs) by email
  • Confirm with IPS whether travel to scheduled appointments is required
  • Inform IPS if you wish to have your appointment rescheduled

Liaison (Host Site/First Nation/Town) Responsibilities

  • Assist evacuees in contacting Indigenous Services Canada (ISC) for:
    • Medical travel arrangements
    • Assist evacuees in contacting IPS for appointment confirmations or cancellations. 
  • Help communicate travel needs to ISC when required

Indigenous Services Canada (ISC) Responsibilities

  • Arrange and provide transportation for medical appointments from host sites
  • Coordinate travel based on information received from: with the
    • IPS, or
    • The patient/host site liaison directly

Ininew Patient Services (IPS) Responsibilities

  • Maintain confidentiality (no sharing of appointment lists with ISC)
  • Attempt to contact evacuees with appointment details when available
  • Work with patients to:
    • Confirm need for travel
    • Notify ISC so transportation can be arranged

Weeneebayko Area Health Authority (WAHA) Medical Transportation Responsibilities

  • Inform clients of the medical transportation process
  • If the appointment is within the WAHA catchment area, provide travel arrangements
